health services have been around for centuries, but it
wasn't until Medicare was enacted in 1965 that home health
services and agencies certified to participate in the
Medicare program grew at an astounding rate. Therefore,
Medicare needed to come up with a control for the growing
number of claims being submitted for home health care. The
HCFA 485, 486, and 487 were developed as vehicles for
submitting a plan of care and patient information to
Medicare or other insurance carriers who chose to use the
forms. Our forms are printed in black ink on high quality
